Before submitting your evaluation, please ensure your form is filled out completely and has been signed.

• You will need to turn in your evaluation and your OJT hours before midnight on the 10th of each month. This is for the previous month you have worked.
• Missing or late evaluations may result in an apprentice having a disciplinary pay delay and with multiple violations, the apprentice may be called to appear before the JATC.
• If your foreman is the same person as your journeyman, have them sign and print their name in one section and write “SAME” within the other.
